Lion Air JT610 crash: What the preliminary report tells us

In testimony before the Indonesian Parliament last week, Utomo the investigator, said that the anti-stall system had activated on the plane as it flew into Jakarta the night before the crash, but the pilots managed to shut it off.

Citing information due to be released as part of preliminary findings prepared by Indonesian investigators, The New York Times and The Seattle Times reported that the pilots fought for 10 minutes to keep the Boeing jet's nose from being pushed downward, due to incorrect sensor readings.

Although the initial report doesn't assign definitive cause to the accident, the NTSC was blunt about the airline's maintenance: "In our opinion, the plane was no longer airworthy and it should not have continued", Nurcahyo Utomo, the NTSC's aviation lead, told reporters at a press briefing on Wednesday in Indonesia.

But Utomo said that it was still "too early to conclude" whether this issue with the system was a contributing factor to the fatal crash, Reuters reported.

Pilots at American Airlines and Southwest Airlines complained this month that they had not been given all information about the new system on the MAX.

Boeing was accused by some pilots in the USA of not properly documenting the nature of changes made to the MCAS system in the 737 MAX, an aircraft that only entered service in 2017.

Boeing said existing runaway trim procedures-essentially using the stabilizer trim cutouts-could address these problems.

Investigators initially suggested the aircraft which crashed had an issue with its angle of attack (AOA) sensors.

Apart from the stick shaker, the pilot of the Denpasar flight noted several other warning signs that appeared on the flight display, including one concerning the difference in the indicated airspeed.

Lion Air CEO Sirait said the airline would comply with KNKT's recommendations which included ensuring pilots made the proper decision on whether to continue a flight. Investigators say the plane hit the water at 450 miles per hour.




After the crash, Boeing pilots expressed concerns that they weren't fully informed about the new system known as the manoeuvring characteristics augmentation system, or M.C.A.S. - and how it would require them to respond in case of the similar emergency.

An AOA sensor provides data about the angle at which air is passing over the wings and tells pilots how much lift a plane is getting.

"We haven't found the information in the manual relevant to the new feature to the 737- MAX, related to the feature for the stall prevention system", he said.

The parents of one passenger is suing Boeing, alleging the 737 MAX 8 had an unsafe design.

The doomed plane's flight data recorder showed that pilots had repeatedly tried to correct its nose from pointing down, possibly after erroneous data from AoA sensors was fed into a system that automatically adjusts some of its movements.

"There were four flights that experienced problems with the airspeed indicator", Tjahjono said.

Overall, Indonesia's aviation safety record has improved a lot since the days when even its national carrier Garuda was blacklisted from European and USA airports over safety concerns.

Boeing's statement did not make any reference to a revised anti-stall system introduced on the 737 MAX which USA pilots say and Indonesian investigators say was missing from the operating manual. Because the MAX aircraft have heavier engines, the center of gravity is biased more forward than on previous models and MCAS is meant to improve pitch feel and provide stall protection.

The Indonesian investigation is continuing with help from US regulators and Boeing.
