Passengers say pilot of ill-fated Southwest Airlines flight is a hero

The Federal Aviation Administration (FAA) said its order for engine inspections would be issued within the next two weeks. CEO Gary Kelly said there were no problems with the plane or its engine when it was inspected on Sunday.

'This fan blade was broken right at the hub, and our preliminary examination of this was there is evidence of metal fatigue where the blade separated, ' said NTSB chairman Robert Sumwalt.

The NTSB is focusing on preventive measures, Sumwalt said. A spokeswoman said it was a visual inspection and oil service of the engines.

What do we know about the engines?

According to Boeing, the CFM56-7B engine is used on more than 8,000 Boeing 737 jet aircraft.

On Tuesday, the engine on another Southwest jet exploded over Pennsylvania and debris hit the plane. Investigators are now working to understand how the window came out of the plane.

In a statement, CFM expressed its condolences to the family of the woman who died.

Riordan, a Wells Fargo bank executive and mother of two, died later at a Philadelphia-area hospital.

Shrapnel from the shredded engine smashed the window next to her causing rapid decompression in the cabin that almost blasted her out of the jet.




Passengers pulled Jennifer Riordan's upper body back into the cabin and started CPR. Seven others were treated for minor injuries.

Asked if the engine failure was cause for concern about the reliability of the overall fleet of 737NG-series airliners powered by CFM56 engines, Sumwalt said: "We are very concerned about this particular event".

Who was the pilot?

One of the most infamous cases of engine failure - a worst-case scenario - involved United Flight 232, a triple-engine jetliner carrying 11 crew and 285 passengers from Denver to Chicago in 1989. First Officer Darren Ellisor was working on Flight 1380 Tuesday when the aircraft suffered some sort of explosion. "Our hearts are heavy", the pair said.

How did the drama unfold?

The flight tracking website FlightRadar24 estimated the Boeing 737-700 descended from 31,684 feet to about 10,000 feet in a little over five minutes. In his post, Laurie stressed that passengers should cover both their mouth and nose with the oxygen mask during an emergency.

Sumwalt said the pilots were able to straighten the jet within seconds after going into a 41.3 degree angle - something not common at all.

She told air traffic control: "So we have part of the aircraft missing, so we're going to need to slow down a bit".

He repeats, "Injured passengers".

"No, it's not on fire, but part of it's missing", Capt Shults replies.
